Dear Providers:
Related to our efforts to ensure appropriate services and supports are offered to older youth with complex needs transitioning out of foster care, we are pleased to share that it has been determined that DFCS foster homes and CPA foster homes can be converted to DBHDD host homes for adults to provide continued care for an older youth aging out of foster care without losing or impacting their foster home status.
If you encounter any barriers to converting a foster home to a host home in order to allow an older youth in foster care to remain with the same caregiver post age 21, please contact Dana Carroll, Deputy General Counsel.
