Dear CPA Providers:

 

Some providers have reported issues with the "Couple Caregiver Re-evaluation Packet Rev. 5.21.pdf" not allowing data entry for the secondary caregiver in the Caregiver Feedback Survey section (p. 20-24 in the original). Instead, it repeated the information from the caregiver who completed the previous section (p. 15-19).

 

We've updated the packet to include a separate file for the secondary caregiver's re-evaluation feedback. You can download that secondary caregiver form by itself, or an updated copy of the packet containing the separate file (14 MB .zip file).

 

Remember that CPAs have until August 1, 2021 to purge prior forms. After August 1, 2021, the expectation will be that only these forms are utilized.
